In the center. You place it far forward for driving and gradually more toward center as you get down to your putter.
You kinda want to place it inside your left instep (for righty) to promote topspin upon contact.
In the center. If you put it off your left heel, you’ll have the tendency to pull your putts. Off the right heel you’ll push them to the right.

I’d go with in the front of your stance and I don’t agree with the idea that you push or pull them when it’s up there. And it does create topspin.

The way I putt.
The ball placement for putting is usually a matter of preference and comfort. Watch the pros and you will see many variations of the stance. Just practice until you find the one which suits you. It may be the Nicklaus crouch, the Raymond Floyd upright, the Tiger Woods slightly open stance which you can copy. Try to develop your own style. This will be the best thing you can do to improve putting.
It depends.
Does your putter have a conventional 4 degrees of loft or does it not have any like a center shaft? If you have 4 degrees of loft you might want to play it in the middle because it de-lofts it automatically. If you have no loft you will want to play it slightly foward
This also is an old argument between teachers, but whatever rolls off the face well without bouncing or smashing it in the ground is what you should use.
